How to submit an assistance request.

If you are a caseworker requesting assistance for a veteran.

1. Submit request form. Download the Agency Request Form here
2. All questions on the request form must be answered.
3. Supporting documents of the need must be submitted.
4. Funds are only provided to the need, never to the individual.
5. Email the form and required documents to vetsrequest@gmail.com
6. If you are a veteran and working with a caseworker, please ask them to follow the procedure listed

If you are a veteran in need of assistance not working with a caseworker

1. Submit request form. Download the Veterans Request Form here
2. All questions on the request form must be answered.
3. A copy of your DD214 or proof of service. You must submit that information to have your service verified by a third party.
4. Supporting documents of the need must be submitted.
5. Funds are only provided to the need, never to an individual.
6. Email the form and required documents to vetsrequest@gmail.com
